All Weiser High School athletes are required to complete and return an Athletic Packet EACH YEAR. All forms of the packet must be completed (signed and medical approval, from Dr., parents, and student) before the first day of practice.
If the athlete is going to "FREE PHYSICAL" night, the physical paper needs to be signed by parents/students, first.
